Universal Medical Form & Universal Permission Slip
The purpose of this form has two reasons. First is to have the medical information all on one form only once and you do have to fill out the permission slip form and medical form every time your daughter or son want to attend an Immaculate Heart of Mary youth ministry event or activity. Second, reason is the unlikely case that you forget to fill out a permission slip for the event or activity this form will be filled out and is acceptable but not preferred.
A regular permission slip will need to be filled out for each event or activity that your daughter or son participates in and this is per the request of our insurance company Catholic Mutual. Permission slips can be found along each of the activity’s descriptions. Please be sure to sign these permission slip forms and turn them in.
